The UCSF Medical Center (University of California, San Francisco) is a University Hospital based in the US. One of their services is an online tool called USCFMyChart, which can be accessed by patients to view their medical records and confidential information.

The website allows its users to carry out a number of activities, such as viewing their hospital records, tests results, refill prescriptions, and even request appointments and view their past appointments and trends in data. Using this service, parents can also view their children’s medical records online. If the children are under 11, parents have complete access, whereas, for children over this age, parents have reduced access.

Requirements to make an account with UCSF Medical Center:

  1. Must be over 18 years of age.
  2. Must have an activation code.
  3. Smooth access to the Internet.
  4. Must be a patient at UCSF Medical Center.
  5. A device that can connect to the Internet.

How to login to UCSFMyChart:

  1. Open your browser and go to http://www.ucsfmychart.org/
  2. Scroll down, and in the fields provided, enter your MyChart Username and Password.
  3. If you cannot remember your Username/Password click the respective link.
  4. Click ‘Sign in’ to complete the Login process.

If you don’t already have an account, just create a new one. If you don’t already have an access code, click on ‘I need an activation code’ on the website, fill out your details, and provide your e-mail address. You will be given an access code within a couple of days.

How to register to UCSFMyChart:

  1. Open your browser and go to http://www.ucsfmychart.org/
  2. Click ‘I have an activation code’ under the Create an Account tab.
  3. In the new window, fill in all the required details and click ‘Submit’.

An app for the online tool is also available on the App Store and Google Play.
